I was sent an application form for one of Krystal Madison's events (Her real name is Crystal Garcia). The thing I noticed from my past online relationship with her is that The Festival of Witches and The Witch of Sleepy Hollow are not business entities. In Short Crystal is doing a DBA. After an extensive records search I have yet to find such filings.
So Why this blog?

I also understand that as long as I remain an employee, contractor or volunteer here, the criminal history records check may be repeated at any time. I understand that I will have an opportunity to review the criminal history as received by Festival of Witches and its agents and a procedure is available for clarification if I dispute the record as received.
I also understand that the criminal history could contain information presumed to be expunged. I hereby affirm that my answers to the foregoing questions are true and correct and that I have not knowingly withheld any fact or circumstances that would, if disclosed affect my application unfavorably. So In short people you are asked everything everything from your SS# and if applicable she would ask you for your blood type ..
Think of this
If you did not know her company is may not by publicaly listed let alone Crystal Garcia is Krystal Madison Why are you giving your information over to someone who already defrauded a Ministry of $800 and apparently others in past venues .
So if this information you feel comfortable giving away why did you not ask for the 3rd party link for the back ground service rather it is sitting on someone's couch ? Yep you just gave your SS# addy and all the rest to a person not a company nor a registered background check company (security company bonded and insured ) This is why she has an Authorization/Waiver/Indemnity.
Congrats folks!!! you just signed away your soul (sort of speak ) time to go to the three credit agencies and check your score and while you are doing this lock up your SS# so no mysterious credit loans taken out with out your knowledge ...
